The applicant is required to print and fill out the Welfare Assistance Application Declaration Form (Applicant) before submitting it to the officers of the Department of Social Welfare (JKM) or the Malaysian Civil Defense Force (APM) at the Temporary Evacuation Center (PPS). If there are any issues, the applicant can refer to the District Office as the Chairperson of the District Disaster Management Committee (JPBD) or the State Secretary's Office as the Chairperson of the State Disaster Management Committee (JPBN)
